Medical History Recordkeeping

The Medical History Recordkeeping Guide was developed by the College to assist Ontario dentists in obtaining the necessary and relevant medical information from their patients to allow for the provision of safe oral health care. The Guide has recently been reviewed and updated to reflect the new 2007 AHA recommendations for the use of antibiotics before dental treatment.

The revised Medical History Recordkeeping Guide is comprised of four parts:
