A person may apply for a Certificate of Registration by submitting a completed application form together with the non-refundable application fee of $100.
Generic Application for Certificate of Registration as a Dentist
If you are or have been engaged in practice in any health profession in another jurisdiction and/or if you are or have been licenced to practise dentistry in another jurisdiction at the time you submit your application for registration in Ontario, then a Certificate of Standing form must accompany your application.
If you are retiring from practice and want to cease being a member of the College, please complete the resignation form below and send it to the College.
