As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Mina as follows: • Dr. Mina must have a complete and up-to-date medical history on file for each patient. Not having a medical history for the patient in this case and then failing to provide an explanation to the College for its absence reflects negatively on him as a professional. • Dr. Mina must inform his patients when root canal therapy he has performed is incomplete and explain to them the possible consequences and available options. • Dr. Mina must document prescriptions he provides to patients and must take into account the medical history of the patient, including allergies and sensitivities, before prescribing medications. In this case, Dr. Mina had not documented the prescriptions he provided to his patient, including narcotics. He also provided medications which were contraindicated for this patient based on the patient’s codeine allergy/sensitivity. • As a member of a regulated profession, Dr. Mina has a professional obligation to cooperate with an investigation conducted by his governing body and to provide his records in full when requested. • The combination of concerns in this matter is serious. Should similar issues arise in the future this panel expects that an even more serious outcome may be imposed.

As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Monir Mina as follows: • Dr. Mina did not perform adequate crown lengthening on the patient’s tooth 34. The failure to do adequate crown lengthening may have resulted in the ultimate failure of the restoration on this tooth, and the need for extraction and placement of an implant. • In addition, the sole pre-operative radiograph was of poor quality; it would have been prudent for him to take additional, and better quality, radiographs, so that he had better information about the condition of the patient’s tooth before making recommendations about treatment including root canal therapy. • Finally, Dr. Mina’s recordkeeping practices were inadequate in that important details were left out of the clinical notes. • The panel cautions Dr. Mina that his treatment of the patient did not meet the standards of the profession in the above-outlined respects. The panel cautions Dr. Mina to ensure he implements the lessons he will learn in the hands-on course in Periodontics that has been ordered, so that these issues do not recur.
