Forms to submit for invoicing

Therapists/counsellors need to complete Form B before invoicing the College for the therapy/counselling they provide to patients who are eligible for funding. This form sets out the legislative requirements for therapists/counsellors and the funding program.

Once Form B has been completed, therapists/counsellors can send invoices to the College. The College will pay the therapist/counsellor directly via electronic fund transfer (EFT).

For all invoices, the therapist/counsellor must also complete Form D. This form tells us if any information the therapist/counsellor provided in Form B has changed.

If therapy/counselling was provided after the alleged sexual abuse, but before the patient applied for funding, the patient may request that the costs they personally incurred out-of-pocket be reimbursed if they are eligible for funding.

The patient would do so by completing Form C. If the patient is eligible, the College would provide the funds to the therapist/counsellor, who would then reimburse the patient, as the legislation restricts the College from paying the patient directly.
