As a result of its investigation of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Nick Danielak as follows: • Dr. Danielak must comprehend the importance of complying with the College’s Standard of Practice: Use of Sedation and General Anesthesia in Dental Practice, and in particular, the obligation to ensure that the three required individuals who comprise the sedation team are present at all times once treatment is initiated under sedation. • Dr. Danielak must have protocols in place at each of the locations at which he practices to ensure compliance with the College’s Standard of Practice: Use of Sedation and General Anesthesia in Dental Practice, and in particular, the obligation to ensure that the three required individuals who comprise the sedation team are present at all times once treatment is initiated under sedation. • Dr. Danielak must practice sedation only in complete compliance with the Standard. • Dr. Danielak must take active steps to ensure that the members of his sedation team understand and abide by the requirement to remain in the room at all times once treatment is initiated.
