Caution

As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Stephen Crosby as follows:

•   Adverse events do occur occasionally in the practice of dentistry, and the way in which a dentist manages these events highlights the level of his or her professionalism. When an adverse event occurs, a dentist must disclose this information to the patient in a straightforward manner; carefully document in the patient chart the event and any follow-up care provided to and discussions with the patient; and take prudent steps to ensure that the patient received appropriate follow-up care.

•    Informed consent requires more than a signed document, but once an informed consent form has been signed by a patient, it should ideally not be modified. If it must be modified due to changes in the planned treatment, Dr. Crosby should ensure that the patient initials and dates his/her consent to the modified treatment.

•    Extraction is non-reversible treatment. Dr. Crosby should ensure that he performs extractions only after taking appropriate diagnostic steps, including appropriate radiographs, documenting his findings, and developing treatment plans including all appropriate treatment options in order for the patient to provide informed consent.

•    As a member of the College, Dr. Crosby has an obligation to ensure that his knowledge of the standards of the profession and expectations of the College remain current, and he must not provide treatment that is not within the standards of the profession, including providing oral moderate sedation when he lacks member authorization to do so.
