As a result of its investigation of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Edward Busvek as follows:

• Dr. Busvek has a professional, legal and ethical responsibility to maintain a complete record of each patient’s dental care. Clear, accurate and up-to-date patient records are essential to the delivery of high quality care. Patient records must be well-organized, legible, understandable and readily accessible, as noted in the College’s Guideline on Dental Recordkeeping (November 2019). 

• Dr. Busvek is cautioned that he must maintain his patient records in such a manner that he is able to access them when requested by a patient, another practitioner, or the College. Original copies of patient radiographs must be retained for the period required by College guidelines and only copies should be sent to a patient’s other treating practitioners.  

• As a regulated health professional, Dr. Busvek is required to cooperate with the College, his regulatory body. Dr. Busvek’s lack of compliance with the College’s repeated requests for records hindered the College’s ability to properly investigate a patient complaint. 

• Dr. Busvek is cautioned regarding his inaccurate transcription of patient chart notes on which he relied in responding to a patient complaint. Dr. Busvek’s communication with the College must be truthful and accurate, which includes the transcription of patient chart entries.
