The panel cautions Dr. Singh to ensure that all of his billings are justified, and that his clinical notes are comprehensive enough to justify his billings.

Caution

As a result of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Durjodhan Raj Singh, as follows: 
• A voluntary restriction is meant to be an opportunity for a dentist to practice and serve patients safely and improve their skills. By not engaging with that process, the Dentist has not shown a willingness to reflect on their practice deficiencies, improve those deficiencies and put patients first. 
• The Dentist breached the public trust by not following the standards of the profession and breaching the signed Undertaking restricting them from providing bridge treatment without the supervision of a Mentor. As stated in the College’s Code of Ethics, the paramount responsibility of a dentist is to the health and well-being of patients. The Dentist failed to uphold that basic principle. 
• The Dentist failed to communicate clearly, openly and accurately with the College and his Mentor during the mentorship. The Committee cautions the Dentist that he should take this opportunity to reflect on his conduct in this matter in order to correct his communication with his regulator and colleagues going forward. 
• Within a very short time of signing the voluntary restriction on his practice, the Dentist breached the Undertaking by placing a bridge without the Mentor’s supervision. The Dentist failed to take personal responsibility for this breach and tried to assign blame to his staff members and an “administrative process” instead of taking ownership for his regulatory responsibilities.

Caution

As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Durjodhan Raj Singh as follows: 

•	The identified concerns are serious, and the College expects the Dentist to implement what they have learned from the previously ordered SCERPs, specifically as they relate to diagnosing, treatment planning, informed consent, recordkeeping, and billing practices.
•	Prior to placing a crown, the Dentist must ensure they have completed a thorough assessment and diagnosed all pathologies. The College expects the Dentist to carefully consider whether the tooth would benefit from a crown based on those clinical findings or whether another treatment option might be better. 
•	Prior to providing any treatment, the Dentist must personally ensure the patient is fully informed of the prognosis of the tooth, all treatment options, and all risks, benefits, and potential complications of the treatment options.
•	Billing should reflect only those services directly provided by the Dentist or where the service is provided under proper, authorized delegation.
