As a result of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Alireza Firouzeh as follows: • As a dentist in Ontario, you have legal, professional and ethical responsibilities to ensure that your billings are accurate, and that your patient records are accurate and complete and fully support your use of procedural codes. The Committee was very disappointed to see a pattern of improper use of procedural codes and inadequate documentation in your records. • You should reflect on the seriousness of the Committee’s concerns in relation to your billing practices, and diligently apply to your practice the lessons learned from the courses you completed so that this issue is not brought to the Committee’s attention again.
