As a result of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Mark Alexis as follows: • In the future, should personal or health issues arise that impede Dr. Alexis’ ability to safely, competently or ethically practice his profession, it is incumbent upon him to recognize as much in a timely manner and proactively step aside from his practice until such time as the issues have been resolved. • Dr. Alexis must be diligent and very particular about his billing practices such that what he bills for clearly corresponds with treatment actually performed for every patient. Further, treatment performed must be driven by a patient’s dental needs and best interests supported by the patient record and not driven by financial gain.

As a result of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Mark Alexis as follows: • Based on the information in this investigation, it appears that during the period of time reviewed, Dr. Alexis took x-rays very rarely while purporting to treat tooth decay, including interproximal tooth decay. It is unacceptable for a dentist to be purporting to restore teeth at the frequency that Dr. Alexis was doing so, without x-rays. In many cases, decay cannot be properly diagnosed and restorative work cannot be properly performed without x-rays, especially when interproximal caries are involved. • Dr. Alexis is cautioned that a dental professional must maintain complete records, and maintain them securely. He is cautioned to ensure that he obtains and documents all of the information necessary to provide appropriate care, including a patient’s medical history, periodontal charting, radiographs, diagnoses, recommendations and treatment planning, informed consent discussions and forms, and financial information including ledgers with procedure codes. • Dr. Alexis is cautioned to incorporate into his practice the lessons that he will learn in the courses in Restorative Dentistry and Ethics that have been ordered.
