As a result of a report, the Inquiries, Complaints and Reports Committee decided to caution Dr. Steven Siqin Zheng as follows: • He must actively and diligently ensure that he is complying with all COVID-19 protocols in place to ensure the safety of the public, including: o ensuring that his HVAC measurements are taken accurately and that his associated fallow time calculations are done accurately and in accordance with current guidance; and o that N95 masks used by staff in the office are appropriately fit-tested as required to ensure safety of staff and patients • He must recognize the position of trust he is in as a healthcare provider, particularly during the pandemic, and be sure that he puts measures in place that protect his staff and patients and earn him the trust of the public

- During Level 1 Clinical Supervision, the Clinical Supervisor will: a. Be available to Dr. Zheng at scheduled times, in-person or electronically, to discuss his implant dentistry; b. review at least 15 of his chart entries for implant dentistry each month and discuss any recommendations with respect to his implant dentistry. If fewer than 15 chart entries are available for any given month, the Clinical Supervisor will review all the chart entries for implant dentistry for that month. If he does not provide any implant dentistry in a given month, no chart review is required. c. Provide a report to the College every three months in the format specified by the College; and d. Prior to recommending that Dr. Zheng practise without Clinical Supervision: • conduct chart reviews for at least 15 patient visits over at least a three-month period; and • confirm that both the Clinical Supervisor and he are confident that he can practise implant dentistry autonomously without a Clinical Supervisor.

If fewer than 15 chart entries are available for any given month, the Clinical Supervisor will review all the chart entries for implant dentistry for that month. If he does not provide any implant dentistry in a given month, no chart review is required. d. provide a report to the College every month in the format specified by the College; and e. prior to recommending that he progress to Level 1 Clinical Supervision: • conduct chart reviews for at least 15 patient visits over at least one month; and • confirm that both the Clinical Supervisor and he are confident that he can practise implant dentistry autonomously without regular input from the Clinical Supervisor.

- Retain, at my expense, a qualified dentist, approved by the College, to act as my Clinical Supervisor. Ensure that my Clinical Supervisor provides the College with a signed copy of the Clinical Supervisor’s Undertaking. Practise under the Clinical Supervision at level 3, as specified in the Clinical Supervision Framework, Dr. Zheng understands that the Level of Clinical Supervision will not be reduced and the Clinical Supervision will not end without the recommendation of the Clinical Supervisor and the College’s prior approval in writing. If the College approves a reduction in the level of supervision, Dr Zheng will continue to practise under the Clinical Supervision at the prescribed level as specified in the Clinical Supervision Framework. Undertake to ensure that the Clinical Supervisor completes all the requirements of the Clinical Supervision set out below, including providing required reports to the College in a timely manner. During Level 3 Clinical Supervision, the Clinical Supervisor will: a. be physically present in Dr. Zheng's practice location and be immediately available to him whenever he is providing implant dentistry; b. review at least 15 of his chart entries for implant dentistry each month and discuss with him any recommendations with respect to his implant dentistry. If fewer than 15 chart entries are available for any given month, the Clinical Supervisor will review all his chart entries for implant dentistry for that month. If he does not provide any implant dentistry in a given month, no chart review is required. c. provide a report to the College every month in the format specified by the College; and d. prior to recommending that he progress to Level 2 Clinical Supervision: • conduct chart reviews for at least 15 patient visits over at least one month; and • confirm that both the Clinical Supervisor and he are confident that I do not require real-time input from the Clinical Supervisor onsite in relation to implant dentistry.

Implant Dentistry • 1:1, evaluative component • A review of the College’s Guideline “Educational Requirements & Professional Responsibilities for Implant Dentistry” • Diagnosis and treatment planning • Case selection • Implant design and selection • Prosthetic design • Occlusal considerations • Material selection • Principles of osseointegration • Indications and contraindications for implant dentistry • Appropriate imaging including use of CbCT • Diagnostic records and case work-up • Use of surgical and radiograph guides • Interdisciplinary communication, when to refer to a specialist and associated referral protocols • Implant success, survival, and failure, including maintenance and follow-up • Diagnosis and management of peri-implantitis • Associated recordkeeping and informed consent ? Implant Dentistry • 1:1, evaluative component • A review of restorative components • Prosthesis design, including cement vs screw retained crowns • Immediate vs. delayed loading • Splinting implants • Temporization • Evaluation of component fit and occlusion • Management of prosthetic and soft tissue complications\ • A review of surgical principles • Disclosure of adverse events • Consideration for use of natural tooth/implant prosthesis ? Dr. Zheng will provide the College with proof of their successful completion of the courses within six [6] months of the date they sign this Undertaking. If suitable courses are not available within [6] six months, Dr. Zheng will notify the College before the deadline and request a date extension or approval of a substitute course(s).

- Dr. Zheng undertakes to restrict his practice such that he will not practice implant dentistry. He will not: ? Perform or provide any aspect of dental implant treatment, including but not limited to: • Preliminary evaluation and pre-surgical assessment, diagnosis, treatment planning, case selection, establishing a surgical prescription, implant design and selection • Obtaining informed consent • Execution of surgical placement of implants • Post-surgical follow-up and pre-prosthetic assessment • Restoration of dental implants, including but not limited to crowns, bridges, splinted crowns • Long-term follow-up and maintenance of previously placed implants and/or implant-supported prostheses or restorations • Management of implant-related complications For further clarity, this restriction on implant dentistry includes any and all new and in-progress implant cases. This restriction will constitute a term, condition and limitation on Dr. Zheng's certificate of registration. The restriction on the certification of registration will remain in place and he will only resume providing implant dentistry after the College is satisfied that they have successfully completed the course work set out below and retained a Clinical Supervisor, beginning at level 3 Clinical Supervision. Dr. Zheng will only resume providing implant dentistry with the College’s written approval and under the supervision of the Clinical Supervisor as set out in the College’s Clinical Supervision Framework attached to this Undertaking as Appendix “A”. If Dr. Zheng is unable to obtain a Clinical Supervisor acceptable to the College on the terms set out, Dr. Zheng will restrict their practice such that he will not perform implant dentistry until he has obtained a Clinical Supervisor acceptable to the College. This practice restriction will remain in place until the College has notified Dr. Zheng in writing that it is satisfied that he has completed all the Remedial Education.
