Voluntary Monitoring of Practice (Office Visits)
Acknowledge that he shall submit to, and not interfere with, unannounced inspections of his practice location(s), by the College or a College representative, including but not limited to inspections of the patient logs maintained by the Workplace Monitor, in order to monitor his compliance.

Undertake to retain a monitor, pre-approved by the College, within seven calendar days.

Undertake to only practise dentistry under the direct supervision and while in the continuous presence of the Workplace Monitor to ensure that Dr. Illango complies with the terms of the Acknowledgment and Undertaking.

With respect to the Workplace Monitor, Dr. Illango agrees that:
I. The Workplace Monitor may not be currently employed by him;
II. The Workplace Monitor must be a regulated health practitioner in good standing with their regulatory body;
Ill. The Workplace Monitor must be acceptable to the College and approved by the College in advance to act as the Workplace Monitor;
IV. The Workplace Monitor must directly supervise him and be in his continuous physical presence on site at any location where he practises dentistry;
V. The Workplace Monitor must maintain a log of patients that he treats and/or who are present in his operatory. The log must be prepared in a form acceptable to the College and may be reviewed by the College upon request;
VI. The Workplace Monitor must report to the College monthly about his compliance with the terms of the Acknowledgment and Undertaking and provide a log of all patients seen by him that month;
VII. The Workplace Monitor must report to the College immediately (i.e. within 48 hours) if the Workplace Monitor believes that Dr. Illango may have breached any of the terms of this Acknowledgment and Undertaking.
VIII. If the working relationship with the Workplace Monitor is terminated for any reason, he undertakes to immediately (i.e. within 48 hours) notify the College, both verbally and in writing, of that fact and to immediately cease practising dentistry until another Workplace Monitor has been approved by the College
and has completed an acknowledgment and undertaking. If he is required to cease practising dentistry as a result of this, this will constitute a term, condition and limitation on his certificate of registration and such term, condition and limitation shall be included on the College's public register.

Undertake to post signage, pre-approved by the College, that shall be visible in the office at all locations where he practices and visible to patients in any operatory where he works advising that he is practising under the continuous physical presence of a Workplace Monitor.
