Caution

As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Singh as follows:

• Dismissing a patient in a worsening state with a compromised airway to present herself to a hospital on her own is not a responsible response. It is important to recognize when a patient’s condition is serious enough to warrant more direct intervention than advising them to find their own way to a hospital. In such situations, you should take more steps to ensure the patient’s safe arrival at the hospital such as arranging transportation for them. Further, in such situations you should advise the hospital of the patient’s imminent arrival to expedite their treatment there rather than simply providing an informal note for the patient to present upon arrival. 

• It is unacceptable to document an appointment over a week after it occurs and to neglect noting in the chart the actual date of the event(s) and/or an explanation for the delayed entry. This is particularly so when the delayed chart entry pertains to a significant event involving the referral of a patient to the emergency department of a hospital. Further, it is unacceptable to submit records with a delayed entry as described to the College in response to a complaint without making any mention of that entry being late and explaining the reason for it.
