As a result of its investigation of a formal complaint, the inquiries, Complaints and Reports Committee decided to caution Dr. Aura Hidalgo as follows:

•	Dr. Hidalgo must keep thorough and detailed records. She is reminded that her recordkeeping continues to fall below the expected standard of her profession. The panel is troubled that despite previous remediation the lessons learned have not been applied. The panel anticipates that her recordkeeping practices will improve following the completion of a more elevated course in recordkeeping, as required in the outcome of another matter. 

•	Records must be maintained in their original form, without alteration. When creating a point of clarification or an addition to an original record, it must be clear that the purpose is that of an addendum which includes the date of the addendum and the identity of the source or author.

•	Dr. Hidalgo is cautioned that before initiating treatment, she must ensure that she has obtained the patient’s informed consent to treatment. As part of that process, the patient must be provided with information regarding the diagnosis, the nature and purpose of the proposed treatment along with the risks and benefits of such treatment, the treatment alternatives available along with the associated risks and benefits, the likely consequences of not having the treatment, and the cost of each treatment option.

Caution

As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Aura Hidalgo as follows:

•	Dr. Hidalgo has a professional, ethical and legal responsibility to maintain a complete chart and record documenting all aspects of each patient’s dental care. This responsibility includes the proper labeling, dating, and documentation of a practitioner’s interpretation of her radiographic and diagnostic findings. Yet, despite a longstanding history of concerns related to her recordkeeping practices and attempts at remediation, Dr. Hidalgo continues to fall below the standard required of her in this area of practice, which raises concerns about her ability to show insight into her past conduct. 

•	Dr. Hidalgo is cautioned to appropriately use radiographs to monitor the presence of pathology present prior to treatment or complications arising following treatment. The presence of a large lesion requires close monitoring and follow-up, and referral to a specialist when necessary. In addition, Dr. Hidalgo must communicate to the patient about the follow-up and management required.  

•	Dr. Hidalgo is cautioned that radiographs are essential before any surgical intervention, including the extraction of a patient’s tooth.

• Dentists are responsible for the work of dental assistants acting under their direction.

• Dr. Hidalgo must ensure that she provides adequate training and supervision to her dental assistants.

• Dr. Hidalgo must ensure that impressions and models are sufficient quality for the fabrication of accurate crowns, dentures and night guards and that patients receive a satisfactory prosthetic.

• Dr. Hidalgo must keep a complete record of all patient care in each patient’s chart and ensure the records are well-organized, legible and readily accessible.

• Dr. Hidalgo is cautioned to review the College’s dental recordkeeping guidance and ensure she incorporates these guidelines into her practice consistently.
