As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Douglas Joseph Cowdrey as follows:

• It is your professional obligation to ensure that your patient records are in accordance with the College’s Guidelines: Dental Recordkeeping (November 2019). Your patient records should include all diagnostic information, and rationale for treatment. Any person reviewing your patient records should be able to determine what treatment and discussions with the patient occurred. 
• It is your professional obligation to ensure that your informed consent protocols are in accordance with the College’s Practice Advisory: Informed Consent Issues Including Communication with Minors and with Other Patients Who May Be Incapable of Providing Consent (August 2007). Your informed consent discussions must be detailed in the patient records, and in appropriate contexts, you should ensure that written informed consent is obtained.

a. Dr. Cowdrey shall abide by any and all recommendations of the Mentor with respect to his provision of endodontic treatment.

b. To provide to the College, at his expense, an initial report from the Mentor within one month of the Mentor being retained, and a report every two months from the Mentor until such time as the ICR Committee is satisfied that the concerns raised in this complaint have been addressed
