As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Bruce Thomas Gardner as follows: • In order for consent to be informed, the dentist must discuss: the diagnosis or problem noted, nature and purpose of the proposed treatment along with the risks and benefits of such treatment, the treatment alternatives available along with the associated risks and benefits, the likely consequences of not having the treatment, and the cost of each treatment option. Whether the patient consents verbally or in writing, the Dentist must keep a record of the nature of the conversation, the information provided, including material risks and side effects of the treatment and the patient’s decision. The more complicated or risky the treatment is, the more specific and detailed the documentation should be. This included treatment undertaken for strictly cosmetic or aesthetic reasons. • Consent forms are not a substitute for the requirements to obtain consent as described above. They can be a helpful way to reinforce information about the proposed treatment and support informed decision making, but a signed consent form is not consent itself. • Prior to extracting a tooth, the Dentist should ensure that they have the necessary protocols in place to identify the correct tooth for extraction.
